Hi Beverly,
I was your patient when I was pregnant with my daughter, Gianna. I hope you can still remember me. I was referred to you by my husband’s friend who is working in construction.
Will you please give me your insight about the two scenarios below: I know that it has something to do with diet and food. I appreciate your time for this.
Scenario # 1:
The patient is a 38 year old female who complains of weight gain despite strict dieting, chronic constipation, and lack of energy. On examination, her skin is thick and dry. Her blood pressure is elevated. You order a battery of tests, including blood sugar, T4, and TSH. Blood sugar comes back elevated. T4 comes back low. TSH comes back high. How do you explain these findings? Note, these problems needn’t be explained by one problem or disease process, but if there is more than one problem be sure to explain how they might be related.
Scenario # 2:
The patient is a 17 year old female Type I diabetic, admitted to the hospital for blood sugar over 300 (3 times normal) and dehydration. She and her friends have been dieting together. They have been dieting on some sort of “eat only meat diet” program that was recommended by an actress who regularly appears on television. What’s wrong – both short term and long term – with a diabetic eating a high protein, low carb diet? (Hint: for the short term problem: What effect would this diet have on other hormones).
